Who we are
Form & Function Osteopathy ("we", "us", "our") operates an osteopathic clinic at 896 Mt Eden Road, Three Kings, Auckland 1024. This policy applies to personal information we collect through our website, our booking system, and in the course of providing osteopathic care.
What personal information we collect
We only collect health information directly relevant to your care, and always with your knowledge.
- Contact details you provide through our website's contact form, such as your name, email address and the content of your message.
- Booking and appointment details you provide when booking through our online booking system (Cliniko), such as your name, contact details and appointment preferences.
- Health information you provide during your assessment and treatment, including your medical history, current symptoms and treatment notes, which we need to provide safe and effective care.
- ACC claim information, where your treatment relates to an accident and you ask us to lodge a claim on your behalf.
How we collect your information
We collect personal information directly from you, whether through our website's contact form, our online booking system, over the phone, or in person during your appointments.

- To manage your appointments and communicate with you about your care
- To process ACC claims where applicable
- To respond to enquiries made through our contact form
- To meet our legal and professional obligations as registered osteopaths

We don't share your information with any other third party without your consent, unless required by law.
- Cliniko, our practice management and online booking provider, which processes appointment and health information on our behalf
- ACC, where you've asked us to lodge an accident-related claim
- Other healthcare providers, such as your GP, only with your consent or where required for your ongoing care
- Google, through the Google Maps embed on our website, subject to Google's own privacy policy
How long we keep your information
Clinical records are retained for a minimum of 10 years, in line with the Health (Retention of Health Information) Regulations 1996. Contact form enquiries that don't result in an appointment are kept only as long as needed to respond to you.
Data security
We take reasonable steps to protect your personal information from loss, unauthorised access, use or disclosure. Our booking and clinical records are held with Cliniko, which uses industry-standard security measures. Contact form messages are sent directly to our email and aren't stored on our website.
Your rights
Under the Privacy Act 2020, you have the right to access the personal information we hold about you and to request that it be corrected if it's inaccurate. To make a request, contact us using the details below.
Complaints
If you have a concern about how we've handled your personal information, please contact us directly so we can address it. If you're not satisfied with our response, you can contact the Office of the Privacy Commissioner at privacy.org.nz.
